Whether you're new to our practice or an existing patient, completing your forms ahead of time means less waiting and more time with your provider. If you are unable to complete the forms, please arrive 15 minutes early.

Yes…and no. For routine, scheduled visits, we are usually able to honor that request. However, for acute same day sick visits, your preferred provider may not have availability. We think it is a good idea to get to know everyone in the practice so that your child feels comfortable with all of our providers. We think they are all pretty fantastic!
Life happens! In order to keep things running smoothly, please reschedule at least 24 hours before your appointment time in order to avoid a no-show fee. This gives us time to offer the appointment to someone else who needs it. If you are more than 10 minute late, you will be considered a no-show. We will do our best to offer the next available appointment.
